Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP)

Harris County Public Health (HCPH) is actively developing the agency’s next Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP).

The purpose of the CHA is to gain a high-level view and understanding of Harris County’s health status, community member concerns, and available resources. The CHIP is the strategy plan for Harris County to improve community members’ health and well-being with solutions to address key health issues identified in the CHA report.

Community Member Survey

Harris County Public Health (HCPH) needs your feedback to serve the Harris County community better. You are invited to participate in the community member health assessment survey. The goal of this survey is to help HCPH understand how Harris County community members view the health status of their community. Your participation is completely voluntary and anonymous. We are collecting survey data from community partners that collaborate with HCPH to improve the health and well-being of Harris County. The goal of this survey is to better understand the role of community partners in the public health system.
