Community Health Improvement Plan

The Community Health Improvement Plan (CHIP) uses data from the Community Health Assessment (CHA) to choose key issues, develop actions, and ensure progress in Harris County. This plan helps local community organizations to set priorities and use resources effectively. It helps make policies and define actions that improve health. The plan includes collaboration with various organizations and opportunities to boost overall health in Harris County.

  • Sharing information about Harris County Public Health and the importance of our activities with community members and key stakeholders.
  • Stronger partnerships with local and state health systems.
  • Ability to identify community strengths and opportunities for improvement.
  • Provide benchmarks for improving public health practices and improving the health and well-being of Harris County.

Top 3 Priority Issues

When the CHA was completed, the Steering Committee voted on the 10 issue statements to select the top three priority issues to build the Community Health Improvement Plan. The three priority issues represent the most significant issues for Harris County to tackle first.

  • Health Insurance & Care Access
  • Mental Health
  • Preventative Care

The Steering Committee organizations will work together to address these issues, guided by the goals, objectives, and activities in the Community Health Improvement Plan. Every five years, the Steering Committee will update the Community Health Improvement Plan to address new priority issues based on new data and the needs of Harris County.
