About the

Community Health Assessment

The Community Health Assessment (CHA) gives detailed information about Harris County’s current health, needs, and challenges. This assessment is used to create plans for meeting the community's needs. The CHA aims to achieve fair health for all Harris County community members by finding urgent health problems and aligning community resources.

To create a shared vision for health, we need to understand our current state of health. We do this by working with various organizations and community members, assessing needs and strengths, and understanding the root causes of health gaps.
