ACCESS Harris County

Accessing Coordinated Care and Empowering Self Sufficiency (ACCESS) Harris County is an integrated care-coordination model that works to improve outcomes for vulnerable individuals through its Care Coordination Team. The team focuses on supporting clients holistically by addressing multiple needs to achieve well-being and self-sufficiency. Housed under the Health Prevention and Coordinated Care (HPCC) Division, ACCESS Harris County works with community members experiencing hardships, including health challenges (physical and/or mental), financial and housing needs, substance abuse, and social inequity. The program provides intensive, wraparound support across Harris County’s safety net system, ensuring comprehensive care and empowerment for residents.

Division Objectives

Improve the health, well-being, sustained recovery, and self-sufficiency of the County’s most vulnerable community members.
Develop and implement a plan to coordinate agency services and reduce duplication of services.
Improve integrated service delivery referrals, access, and sustained engagement of clients.
Develop strategies and policies for improved efficiencies, better utilization, and better outcomes.

Who We Serve

ACCESS Harris County seeks to improve the outcomes and self-sufficiency of Harris County’s most vulnerable populations, including but not limited to:

Youth and adults involved in the justice system, especially those in vulnerable communities.
Birthing parents and their expecting families at risk of complications in prenatal care, post-partum care, and infant-early child development.
Peoplewho are at risk of homelessness or unstably housed, including those dealingwith domestic violence.
Community members who are reentering society following incarceration.
Community members who have one or more chronic diseases, including hypertension, obesity, lung diseases, cardiovascular diseases, pre/diabetes, and diabetes
