The ACCESS Harris County Re-entry Cohort aims to help justice-impacted individuals by providing permanent supportive housing, employment assistance, food assistance, GED preparation, job training, health care, and more by providing full wraparound services.
Eligible individuals that are justice-impacted work with a case manager on a one-on-one basis. The participant's case manager and care coordination team works to ensure needs are met and goals include reducing recidivism, increasing compliance with supervision programs, reducing substance abuse, and increasing participation in educational and/or trade programs.
