ACCESS Harris County

Accessing Coordinated Care and Empowering Self Sufficiency (ACCESS) Harris County is an integrated care-coordination model that works to improve outcomes for vulnerable individuals through its Care Coordination Team. The team focuses on supporting clients holistically by addressing multiple needs to achieve well-being and self-sufficiency. Housed under the Health Prevention and Coordinated Care (HPCC) Division, ACCESS Harris County works with community members experiencing hardships, including health challenges (physical and/or mental), financial and housing needs, substance abuse, and social inequity. The program provides intensive, wraparound support across Harris County’s safety net system, ensuring comprehensive care and empowerment for residents.

Who We Serve

ACCESS Harris County seeks to improve the outcomes and self-sufficiency of Harris County’s most vulnerable populations, specifically the following cohorts:

  • Youth and adults involved in the justice system, especially those in vulnerable communities served by Harris County Public Health’s Community Health and Violence Prevention Services Division (CHVPS).
  • Black birthing parents and their expecting families at risk of complications in prenatal care, post-partum care, and infant-early child development.
  • People who are homeless, with physical and/or behavioral barriers, and those dealing with domestic violence.
  • Community members who are reentering society following incarceration.
  • Young adults who are aging out of the foster youth system.
  • Latino community members who have one or more chronic diseases, including hypertension, obesity, lung diseases, cardiovascular diseases, pre/diabetes, and diabetes

Division Objectives

  1. Improve the health, well-being, sustained recovery, and self-sufficiency of the County’s most vulnerablecommunity members.
  2. Develop and implement a plan to coordinate agency services and reduce duplication of services.
  3. Improve integrated service delivery referrals, access, and sustained engagement of clients.
  4. Develop strategies and policies for improved efficiencies, better utilization, and better outcomes.
  5. Develop an integrated data sharing system to support care coordination across agencies.

Funding Source

The ACCESS Harris County budget includes funding from the American Rescue Plan Act (ARPA),the Harris County general fund, and the Hope Grant.
