The Health Prevention and Coordinated Care (HPCC) Division was launched in 2025 to improve health outcomes and promote equity by connecting Harris County residents to essential prevention services, public benefits, and social services. By integrating programs such as ACCESS Harris County, HCPH Connect, and Chronic Disease Prevention, HPCC addresses community needs through a comprehensive and collaborative approach.
