Community Health and Wellness Services
The goal of the Harris County Public Health (HCPH) Community Health and Wellness Platform is to implement a community-based health services program which includes mobile health (curbside) services and community health and wellness centers to provide affordable, convenient, high quality healthcare services to Harris County residents most in need. This mobile platform allows HCPH to address the access to care barrier by bringing healthcare services and resources directly into communities via Mobile Health Impact Teams. This new platform allows HCPH to work in conjunction with our brick and mortar health centers to expand our reach to improve health outcomes of Harris County residents.
This project will improve access to appropriate and affordable wellness visits for target populations through the provision of community-based health services. A proportion of Harris County residents are uninsured, and many lack the resources or face geographic and logistical barriers in accessing basic health care services. This project seeks to intervene within HCPH jurisdiction by delivering high quality healthcare services, and track referral services offered to communities and residents directly.
