The COVID-19 Outreach team of Harris County Public Health is a diverse team of Community Health Workers with a mission to educate, encourage, and empower our community members to prioritize their health and wellbeing. The goal of our team is to provide COVID-19 education and resources throughout Harris County in order to decease vaccine hesitancy and increase awareness of current and accurate information about COVID-19 vaccines, testing, and community resources. Our team continues to grow, adapt, and respond to the community’s needs throughout the pandemic. With a data-driven approach, our team aims to bring crucial COVID-19 information, valuable resources, and social services to our communities that need it the most.

Community Impact and Forms of Outreach

Our Community Health Workers engage with their fellow community members in the following ways:

Community events

  • We partner with businesses and organizations to engage in public spaces such as grocery stores, bakeries, flea markets, health fairs, food distribution sites, bars, and more!

  • We distribute COVID-19 Care Kits which contain updated guidance on prevention, testing, and vaccines/boosters as we continue to learn more. Our materials are available in 9 different languages: English, Spanish, Vietnamese, Korean, Chinese Simplified, Chinese Traditional, Arabic, Urdu and Hindi.

In-person and virtual presentations

  • We share the latest COVID-19 updates, COVID-19 vaccine facts, and information on how to get connected to additional community resources. We have staff members that can present information in English, Spanish, Urdu, Hindi, Igbo, Bengali, and French.

Community canvassing and phone calls

  • We share crucial information on free COVID-19 testing, vaccines, and additional community resources to local businesses, organizations, faith-based centers, apartment complexes, mobile homes, etc. in-person and over the phone.

On the horizon

  • We have been granted funds from the Centers for Disease Control and Prevention to expand our work in the community in preventing COVID-19 and poor health outcomes. Some of the upcoming projects are:
      • Community Townhalls, sharing COVID-19 updates and hear feedback directly from our community members.
      • A Monthly Newsletter, sharing important updates, trends, and community resources.
      • Community Health Ambassadors, who will work alongside our team in getting the most important COVID-19 information and community resources to their own communities.
      • Community Health Worker Conferences, to develop local Community Health Worker’s professional skills and learn from leaders in the field.

HCPH Connect is a team of state-certified Resource Navigators that assist Harris County residents to apply for state benefits and get connected to additional resources. Become a Community Health Leader!

Do you want to spark change in your community and earn an incentive? Interested in learning about your local public health resources and programs? The HCPH Outreach Team needs your assistance to inform, encourage, and promote positive health behavior and resources from within your community. 

HCPH Community Health Leaders are comprised of small cohorts of 20 people who are resilient, empathetic, and driven to provide up-to-date public health resources to the residents of Harris County. By the end of the program, you should be prepared to educate and have meaningful conversations with your fellow community members on public health concerns and connect individuals to needed resources and HCPH programs. Upon completion of the program, Community Health Leaders will receive an incentive of $300-$325.

Following the submission of this form, applicants will go be contacted for an interview & selection process. Due to the limited cohort class (20), chosen applicants must meet the following Eligibility Requirements:

Eligibility Requirements:

  • 18 years or older
  • Harris County Resident
  • Can commit to three monthly workshops (2 hours each) from July-September (dates provided in the application)
  • Can join in person
