HCPH Health Information Services (HIS) is responsible for the maintenance of health information, including medical records, for the HCPH Clinics. HIS works to maintain a high quality professional health information management (medical records) system that meets all applicable state and federal laws. HIS ensures that medical records are handled with confidentiality in accordance with Health Insurance Portability and Accountability Act (HIPAA) provisions.

To Request Copies of Birth or Death Certificates

Harris County Public Health does not keep birth or death certificates. To request certified copies of birth and death records for births and deaths within the City of Houston, please contact:

To Request Copies of Immunization Records

Harris County Public Health (HCPH) keeps immunization records for vaccines given at HCPH Clinic Locations, as well as immunizations given by Texas health care providers using ImmTrac (Texas Immunization Registry).

Copies of Immunization Records/Certificates may be obtained by calling the appropriate HCPH clinic, or by calling Health Information Services at (713) 439-6025.

To Request Copies of Clinic Medical Records

Harris County Public Health (HCPH) keeps only Medical Records for clinical services provided at HCPH clinic locations.

Medical records may be obtained by calling HCPH Health Information Services at (713) 439-6025. Please be prepared to present a valid photo ID. Protected health information cannot be released without: 1) a valid photo ID, and 2) a signed Authorization for Release of Protected Health Information Form. Note: Medical records cost $25 for 1-10 pages, and $1.63 for each additional page.

Medical records for services provided at any Houston Department of Health and Human Services location (inside the City of Houston) may be obtained by calling (713) 837-0311.

Medical records for services provided at Harris County Hospital District hospitals (Ben Taub, LBJ, and Quentin Mease) and Community Centers may be obtained by visiting their medical records website.
